How do I post a review on a provider that got all of the requested info to include a copy of my work ID and then just stopped communicating and left me hanging? She has good reviews and a stated screening process but I complied with all and would have appreciated a "I won't see you" text vs just leaving me hanging until it was too late to schedule another appt.

You don't post a review. Nothing happened / there is nothing to review.
You can, however, post your account of what transpired in the Co-Ed section so as to warn other guys to stay away.

Oh, and never EVER give a provider your "work ID".
