My understanding is that to fight money laundering, fraud, and terrorism funding, the Patriot Act requires issuers to verify personal info like name, address, and SSN on all general purpose reloadable cards (GPRs). This includes all so called "stored value cards" exceeding $500 which are reloadable and prepaid.
Most people stick to Visa Vanilla prepaid cards which don't require any personal info, can hold up to $500, and you can use dummy information if you want to register it so you can check the balance and register an address/zip code for online purchases that do AVS checking. Unfortunately they are not reloadable and you can't buy everything with it, particularly vendors with internationally based credit card clearing houses may get rejected.
You can buy some cards online from other countries with less stringent banking laws but I'm not aware of any that are universally accepted in the US. I know there have been some in the past which used to work and were commonly sold on various Bitcoin enthusiast forums, but they seem to get shut down and or frozen as countries clamp down on high tech money laundering.

PayPal is great, but it's not anonymous and they are notorious for freezing "suspicious" accounts.

Ahhhh gotcha!!! Damn because PayPal has been very convenient in my webcam payments and website extras payments as well as ebates fat check payments.. I was not aware of the anonymity of it.

I've experienced problems with WalMart cash cards... It's ran by green dot and they monitor all transactions and payments

Again, it's a good service, but Mango Money is not anonymous. It requires identity verification to set up an account as per the AML laws (anti money-laundering) and other federal financial reporting requirements.

KROGER prepaid debit card.
Costs $3 per $500.
You have to activate it using the phone service. You can register it to mikey mouse, giving a neverland adress and a zip code if you want to use it online.
If you want it refillable then you have to give a real ssn.
If you dont want our curious gestapo wannabe uncle sam snooping into your business. Use it once and dispose it.

Thank all who responded. You confirmed what my research found. But money laundering over $500? I guess if you did 10,000 of them it would make sense. Just trying to help a friend who has an online shopping addiction and not enough claimed income to support purchases made.
