I would think the more specific you could get on getting responses to your areas of concern, the better. Something like this checklist could only help, I'd think.

I think its an interesting alternative. However, as stated earlier some providers may not have the time to fill out or check off a chart. Nevertheless, I think if we truly want to keep our community safe and healthy we should try to help out one another where it is possible to do so. I really liked the point about hobbyist having a tight circle in regards to verifying the legitimacy of a potential provider, thus providers should do what they can to keep us all safe. You may be on to something here :-)

I think its a great idea, but am with others on the fact that many providers may not wanna take the the time to fill it out or not remember much on that particular gentleman. I say keep up with it, something is better than nothing!!

Hey Kendra, great point. Something is always better then nothing. Earlier I had suggested using little stars after the questions so the provider could answer in multiple choice.

Why not ask about the guy then at the end of our reference check say, "If you have the time and remember the details, could you fill this out as well?" That way if she doesn't remember the details or doesn't care to fill out the form, she'll still get back to you without being put on the spot to either answer the form or not respond. As has already been brought up, unless I've seen a gent more than a couple of times recently, I sometimes don't remember the good/mediocre details, only the bad ones.
