It’s not uncommon at all, at least in the service. Anything less than 500 should trigger TCAS alerts, which both of these aircraft have, but it’s useless at low altitude. At least 2 of the 3 (ATC-helo-commuter) fucked up royally for this to have happened, the helo should’ve been nowhere near the commuters glidepath. If I was a betting man I’d say the commuter was doing exactly what he should’ve been doing, ATC blew the assignment and the helo pilot wasn’t experienced enough to realize it. Hopefully we’ll find out how this happened, but probably won’t.
And apparently the tranny thing is fake news. Surprising because I read it in the Daily Mail.
