I blame #2 mostly on the Republicans in Congress. They had 8 years to come up with a replacement plan ready to go when a Republican took the White House. But they completely dropped the ball by dicking around as usual. Fortunately Obamacare is going to collapse on its own with more and more insurance companies pulling out. Which of course is what the Democrats wanted and expected to happen all along so that they could impose a single-payer system at some future point.
