More seriously, I'll be surprised if the marginally greater inconvenience isn't a factor in their limited adoption, especially for ladies who fuck for a living. Especially on the low end of the profession, where the lady really wants the guy to blow and go.
Other factors might be expense (I've never priced them; I'm just spitballing here), and ignorance about their existence or use.
