trying to get verified as a provider..help!!

I sent an email over 3 weeks ago to the email address listed for Kansas City Metro and Kansas to be verified as a provider...I still do not have access...I'm not sure what is going on as I provided all info requested.

Hello, I am on your site as a registered member. I suppose I should be happy an feel more secure that this is not a very user friendly site. But it is frustrating at this point.
