It is a hard question for a complicated business. I can only answer in generalities. A "bad apple" policy is one designed to game the system and fool the marks into thinking they have comprehensive coverage from a reputable, financially stable business. I once inquired into a 10 dollar a month "dental policy". (I was obviously skeptical) It was nothing more than a list of dentists that agreed to take in patients in your area for a negotiated rate, supposedly a discount. It implied you could get your teeth cleaned, x-rayed, and get a free toothbrush for next to nothing. They wanted you to think it was a good policy when it was just a basic list - and they carefully worded things to make you think it was real insurance without actually saying that it was.
Given free reign, I can imagine any number of tricks a health insurance company would pull to deny, delay, and obfuscate.

I think the current fraud and consumer protection laws would take care of that. Certainly we don't need a government takeover of an entire industry to protect people from a phony $10 dental policy.
I just keep hearing about all the sub-standard policies being cancelled. I want to know in what areas they are deficient. The fact that you don't know proves that you, and others, are simply spouting the government talking points, and don't really know what you're talking about.

do your homework and quit begging ... look up the requirements of ACA, compare those to the coverage of policies that are being canceled because their requirements fall short of the ACA requirements ...
Im pretty sure you'll find your sophomoric question is a little more difficult to answer than you think.

Under ObaminableCare there an extremely limited judicial review, which was one of the things I early on posted was disagreeable to me ... because "The Secretary" could "make law" by passing out regs, but the validity and application of those regs were relatively safe from a judge's eyes .. judicial review .. in the absence of a U.S. constitutional claim..

It was a legal offering, carefully worded to not be a policy at the same time it appeared to be one. Legitimate competing businesses get cheated just as badly by crooks pretending to be honest and reputable. Look at the explanation of benefits for every procedure you have, and the ways insurance companies can get to you. The only reason they won't nickel and dime you to death in the future is the government will require uniform coverage and payment standards.
