I think you should PM her on ECCIE, and let her know that you would like to book an appointment, but also let her know that you guys used to know each other without letting her know who you are. This should be enough info for her to decide whether she wants to see you or not. If not, then you haven't lost anything. You can still try to add her on fb later on and rekindle your friendship in real life. But if she says yes, then you can have what you wanted and she can have a good client. But please, don't out her. She won't out you because in doing so she will be outing herself. It is more of a risk for her than for you. Then again if you were able to find her because she is recognizable in her showcase (i.e. maybe she is showing her face), then she might not really care if people know what she does.
