I dont accept or give references if the meeting hasnt occurred in the past 3 to 6 months. I give the client benefit of the doubt after provider okays his reference but that doesnt mean Im at ease until we're both comfortably in a compromising situation that completely confirms legitimacy.
Just because he wasnt an asshole to her doesnt mean he won' t be one to you.
In any case I think a good reference situation would be when :

Client contacts provider and asks BEFORE using her as a reference.
There should be some level of communication between the two parties before The Giving of a reference and the Okaying of one.
