1) ALWAYS at a hotel with decent room service and a bar... this way if it's a bail situation, you can always send her away - or both go. (NEVER leave her the room).
2) If it's 8-12hrs, you have to sleep - and you have to eat... If you want to stay in, then no worries if hotel offers services.
3) I think you don't have to know them well first... but there's always some interaction (texts, calls, etc). If she's too time conscience to be playful - she'll be a clock watcher and it'll be a bad overnight. The best are always interested in "overnights"... It's like MFF, why try to push a provider who's strickly-dickly into it? Better to get someone who WANTS to do it.
4) It's a lot of time. Even long porn movies are only 3 hours. The advantage is the triple "Hit it early evening, middle of the night play, breakfast service".

Other than that - your call. I always do dinners, events, something... and I always ask. (Yes, some are just in it for the money), but if it's a cool event - sometimes it's part of the compensation package; so rates will vary. Always 4-5 star places, and I let them know in advance what's going to be happening. It's more, but as I always say - its a "better value".
Final note: Just my feel on this, but it's easier to get a better deal with high end providers... easier to get from $2k to $1k, than from $600-$500. (But, again - that just might be me).

I can handle a discussion on Eisenstein's Special Theory of Relativity, the Copenhagen model, Heissenberg's uncertainty principle and all the various philosophical implications of those, but I get off the bus when it comes to string theory.
