This will be my first trip to Brazil this summer. I do not speak any Portuguese, and my Spanish is only slightly better than my Portuguese

I will be in Fortaleza, which is not reviewed here.

In the more popular places you will always find someone who speaks English. As for reviews you should go to www.clubhombre.com which has trip reports for all the major cities in Brazil. To join there is really cheap and they have a very active discussion board on all things Brazil. There is no such thing as a typical 1 hour appointment in Brazil. If the girl and you click she may stay hours, even days. There is no such thing as one and done in Brazil. Where will you be staying? If it is a hotel find out if it is "girl friendly" which means you can bring in a guest as long as she registers which is not to say you have to pay more, for everyone's safety hotels that are girl friendly take down their info. When you go to the girl page checkout if there is any information about the language.
