Only had a condom break once, in the civy world not hobby world, probably about 20 years ago. She provided it so not sure how old it might have been. Discovered it changing positions, before I popped.

It happens and sucks! Worse part about the hobby, and guys when you cum why don't you try telling the lady instead of keep going! This makes for the dick to shirk, making it easy for the condom to slip off! A lot of guys do that, I guess because they want it to last longer? I would say condoms break and slip off about 90% slim chance, depending on how much the lady works really matters. Word of advice, if you cum, then your done! Don't try to keep going, it's gross and unsafe!!! I will not see a guy again after he does this, he obviously doesn't know what he is doing.

I always keep my hand down there to make sure, and if a guy has a problem with or don't like it, then too bad! It helps me feel comfortable and I know how easy condoms can slip off! Especially if you have a small short dick, then you have to be extra cautious. Or if you get really wet (which I do) and that makes the condom to move more than it should.

If you over-fill them, they will almost always break...especially if you try to overhand them with your fingers apart...

Also, providers and hobbyist need full std screenings I say at least every 3 to 6 months! We are more active than the norm, that calls for more tests and screenings. Even though we do use protection, we all know they are not 100%.

Um, Let me make myself CLEAR here!
If a guy cums and keeps going as in, he came and is still fucking 5 minutes after. Its NOT safe! I never said I make them pull out when they cum, did I? NO! I said if they keep on going until their cum is oozing out the condom or it makes it fall off!

I get full STD/HIV screenings every 14-30 days. I have to since the top adult studios won't even let you through the door without one. I use to get tested every 3 months just in my normal life, but since I have sex more than I use to I make sure I'm safe FIRST.
I walk around with a print out and PDF copy of my most recent tests
