With two reviews recorded, you should only need to contact your local Moderators in the Houston Area. I *think* they can be reached by email at Houston@eccie.net as that seems to be the standard addressing scheme they use (city name@ eccie address) Or you can PM/write to them individually. There are 6 moderators for the Houston Board, DarthMaul, DickEmDown, mojojo213, SOULMANIKE, Justanotherdude, and Pistolero.

Let them know you have your two reviews here and see if they can't get you set up with Verified Provider status.
