The outer counter is on the thread list in the Forum view. It is the second number from the right under the heading "Replies" (which as Marcus pointed out to my sleepy brain, is the.... Replies (not including the original post))

The "inner counter" is when you are in the thread and you look at the number on the post. For instance this one is #1008
This number counter includes the original post which is #1

And the "inner count" is the highest number/latest post when you click into a thread, so it always starts with one, for the original post. The "outer count" counts replies only, so it is always one less than the "inner count" as it starts at zero before an initial reply post (the second one total) is made in any thread.
