Good question Strober, I have wondered that as well. Here is my history with tipping: I have seen providers all over the country for the past 20 years. If I am paying $400 to $500 for an hour appointment (normal activities - no greek), I don't usually tip because I figured they have factored the tip into their rates. However, if I pay $300 or less for a hour appointment (normal activies) - then I will tip if they are good and are into the session.
My ATF I see in the panhandle charges $300/hour and I almost always tip her because she is so good and always delivers an excellent experience. I usually tip $60 to $100 to her.
I think tipping is something that is nice if you can afford it and the provider gives an exceptional experience. They are service providers, if they provide an exceptional service, tip them if you can, if they only provide the bare minimal service, don't tip.
