Medicare, generally, has been paid for by payroll taxes and recipients working for decades. Obama famously said that each year there was 200 BILLION dollars in Medicare and Medicaid fraud. That was just the fraud. The govt couldn't afford it if they extended it to everybody. It would make the solyent green new deal look affordable.

It would provide a foundation for medical coverage for so many.
Everyone who needs care gets it but the system is supported by private insurance which subsidizes the low priced Medicaid.

I thought everyone already knew that!
