2. There really isn't an effective solution to the cancellation issue. On P411, if you put a cancellation policy in writing and they book via the "Schedule an Appointment" feature, Gina will step in on helping to collect (I believe). But the policy MUST be in writing and must be specific.
3. I couldn't tell you a good answer on this one. I don't collect a fee to prebook-- but I do base my travel decisions on them. In order for me to make the trip, my expenses have to be covered by the prebooks, knowing full well I will generally book more appointments once I've arrived. If I cannot cover my expenses before I leave home, I won't leave home.

If you can make as much as you would at home, and pay for the whole trip, that is a winner. Plus, you can vacation a little on the side.
