Every "duo review" (i.e., review of a threesome (or more) session with two (or more)* providers) I've seen here is one review naming both providers.

If you have additional details to add to your original review (something you forgot, or something the provider didn't want mentioned at the time but has since changed her mind about), PM the mods for the relevant forum and ask them to edit your review to add the desired information.
Like Roaming suggests, if you have had a second session and she expanded the menu or the experience was so different to justify the new review, a second review may be in order. It's up to the mods' discretion to decide if the second review has enough additional information to justify an award of additional PA credit if the review is more recent than the 90** days Roaming mentions. Still, you might decide, "more PA be damned" and post the review anyway if you think it's important to bring this information to our attention.

I'm not sure I follow what you mean here by a review being "assigned" to a provider. If you mean the review being listed in the "ECCIE Reviews" tab of her profile, it's my understanding that this is done manually by the mods and admins (and some mods are better/more timely at it than others), and there's nothing (other than mod/admin laziness or an ECCIE policy I've never seen referenced) that would prevent them from linking the review to both profiles.
