I know that we put both ladies in a single review but I have 2 questions:
a) assuming only 1 credit despite reviewing two ladies. Is that correct?
B) one is a utr civilian, but one is a provider. Assuming that is ok as long as the provider is the "review". Correct

Thanks for the excellent questions.

One credit per review, regardless of the number of participants... correct.

If I understand correctly, you are seeing an established provider and a civvie... the Provider's name goes in the title under provider. You can add "and friend", if you like.
The contact info should be that of only the provider.

If you want to give a nickname for the civvie in the ROS.... e.g. "she introduced herself as "Jane"...", I think that would be appropriate. You can refer to her a "Jane" when writing your ROS comments.... that would seem to be an easier read than to say "civvie friend", as an example.
