How do I post a review for a provider

I have been trying to figure out how to post a review for a provider and have been unsuccessful. Would you please provide guidance? Thanks.

Just visit the appropriate forum (Independent, Spa, Strip Club) and look at the top left of the forum. There is a blue "New Review" button. Fill out the form and submit. After a day or so, the staff will review your review for Premium Access credit. We will also assign the review to the appropriate provider. (Please help with that by clearly identifying her, preferably with a link to her profile on ECCIE if she has one.)
