Bladerunner: Never happened to me.
But the thought that the girl that knocks on the door of my hotel room is (surprise!) my daughter-in-law's kid sister is an interesting fantasy. The sequellae are fun to explore as well.
First would come the mutual surprise/shock, followed by the mental resorting of how we each view the other.
Would we do the session anyway? Dunno.
Would we just agree to never speak about it? What about family gatherings like Thanks Giving?
All intriguing paths to think along.

Never someone I knew previously
but I've seen 2 that I met in session.
One, I used to see pretty regularly and I bumped into her when I was in a convenience store with my brother. We exchanged a look and let it go at that. We joked about it the next time I saw her. She used to start each session by welcoming me into her apartment, then immediately dropping to to her knees and giving me a BJ. She said "At first, I wondered if I should just come over and start sucking your dick - then I thought "Nah, better not". I told her my bro would have been OK with it, so long as he got a BJ, too.
The other, I saw when I was in a home improvement store with my ex. Like the first one, we exchanged a look and let it go at that. I never saw her again, but I had only seen her once or twice.

I've had people text me to see if it was me or not. Most of the time we just ignore each other.
My first social I went to a hobbyist mistook me for their wife's best friend. I was scanning people as they came in and his reaction was priceless. His face twisted into a mask of fear and he literally scurried and hid behind a wall partition. He kept peeking around to look at me. Later, he finally figured out I was not someone he knew and explained his reaction. I laughed it off.
Thankfully, the chances of running into someone I know is very minor. All my family and close family friends are thousands of miles away. The males I know IRL wouldn't hobby.

Twice.
One I knew in real life first. A co-worker and a bit of a friend, but not my type as far as erotic attraction. I found out accidentally that she escorted part time. It was a long time until she kwen that I knew. We occasionally talk about topics of common interest.
The other I had a date with on my first visit to her city. Quite enjoyable evening was had by both. The next day I walk into the business meeting that was my primary reason for being in town, and she turns out to be part of my client's team. We both stayed calm and professional. I worked on the project with her for almost a year and a half and everything was very professional with no play time meetings. She turned out to be a very cool lady--and exceedingly smart. She did some escorting part to pay off school loans, part because she enjoyed the sex. After the project was finished we did see each other a couple times in our original capacity until she got promoted and moved away.

I met with a guy whom I had a crush on in high school, but doubt if he ever noticed me back then. Anyway fast forward several decades and he was sitting on Gigi's couch. He kept on saying, "Now, where are you from?" I just said around the area.
I'm not going back to any class reunions, not because of this - because if I really wanted to talk to friends from then - I would find them and talk to them.
