Port of Call, New Orleans. A big fat burger that has lettuce, pickles, tomato and onion on the side and can be topped with grated cheese (cheddar or swiss), sauteéd mushrooms and bacon bits served with a baked potato that can include the usual toppings (butter, cheese, bacon bits, sour cream and chives) plus the mushrooms.

That's a rather subjective question. Being in Houston, I've never had an in and out burger. I do enjoy Five Guys. Each person's palate is different. For example, my cousin loves Whataburger while I am not a fan.
However I will go with Five guys.

J's Diner in Emporia, KS.
Its a small drive thru/eat in diner. All his burgers are delicious.
His family owns a cattle business as well and all their beef is delivered fresh 3 times a week.
If you dine in they give you a free ice cream cone after wards

Being a native Californian I have to say "In and Out" burger. All fresh ingredients made fresh to order. They even have a secret menu, its pretty good and super yummy.

One of the best burgers I've ever had is in North Dallas, kinda a hole in the wall, but pretty darn tasty and cheap.
I tried some place in Green Bay, Wisconsin...They had butter hamburgers........Tasty

I've had Port of Call and enjoyed it, but it is no where close to a 5 Guys. And this comes from a restaurant manager who works for the same company that owns Smashburger, another high end burger place like 5 guys, and Mighty Fine
