List of (do's & don'ts) for a Gang Bang?

A friend is setting up a gangbang for his wife in January. She has been with her husband and one other man in a MFM. Now SHE wants to have a gang bang with 5 guys. I want to know a list of (do's and don'ts) that I can pass along - ahead of time - to the hubby so he can set some ground rules.

i.e. DP allowed?
i.e. CIM or CIV?
i.e. Rough sex allowed?
i.e. Covered not Covered?

etc, etc, etc.

In a nutshell, the rule of thumb and gangbang etiquette states that none of those things are allowed unless expressly okayed and/or invited. She or her husband (hopefully at her request) will guide you as to what is permitted. But unless told otherwise, keep it like a normal encounter, just with more people. Many guys will have a sword crossing issue, so keep that in mind as well.
