Actually, I throw out a little wad of paper on my way out too. It's half of the piece of paper that had the room number and phone number on it. The second half I throw away at a drugstore or gas station on my way home. (I carefully tear the paper so that the entire phone number or room number doesn't appear on the same half).
I know it's overkill. What would someone do with a person's phone number and room number? I just don't want any link at all in my home trash after reading about the Stanford Law School escort whose trash was filtered through for months while LE was building the case.

I'm equally careful about those papers with hotel room and phone numbers. I'll bring them home and put them in my shredder. I'll do the same with envelopes if they have either the name of the hotel or "Elsa" written on them.

My preference is to provide a nice card inside of the envelope, placing the "gift" inside the card. I never sign the card, and never write anything on the outside. If at all possible, I try to find a card with some humor to it, and one that fits the lady I am visiting. (It seems to take the edge off of the business side of the transaction).
I never thought about eyeing the ladies at they exit the hotel, to see if they discard white envelopes while departing. But, that would not be a bad way to pass some time while enjoying a cup of coffee from Cafe Du'Mond, and some fresh beignets.

I'd never fault a gentleman for not writing my name on the envelope. What if he'd just left the bank and didn't get the chance to write my name on it? It's all good, so long as it's handled tactfully when meeting at a public venue.

The only time I'm going to give it much thought is if there isn't an envelope. Blank or written with a Sharpie, makes no difference to me.
