Ahmadinejad and the Ayatollahs are crazy. They are willing to nuke Israel with no concern about whether they, or anyone else, dies in the process. They are believers in the cult of the Twelfth Imam. They believe nuking Israel will trigger the end of the world, which will cause the return of their Messiah. They believe that when the Twelfth Imam returns, he will bring about a worldwide Caliphate, basically their idea of Heaven on Earth.
