Mal & Inara were never together. They had deep feelings for each other which neither ever acted on. Inara wasn't fond of Mal calling her a whore and Mal wasnt fond of her being one. No history between them prior to the show. Ep 8 "Out of Gas" shows their first meeting was when she sublet the shuttle from him.
As far as companions level courtesans go, they already exist. However as evidenced by ep.13 "Heart of Gold" not all whores in the Serenity universe were Companions. Only the most beautiful, talented and capable made it through training - and had to return to the temple yearly for requals and stuff. According to Inara herself the term "whore" was accurate for non-guild hookers.
