Congress is not. The Grassley amendment explicitly requires Congress and staffers to purchase policies through the exchanges.

Presidential healthcare for more than a century has generally been performed by the military.

They're only "exempt" from it if they aren't required to pay a penalty if they don't have insurance.

Yes, we are all exempt from having to purchase health insurance. You get a fine but you are not required to pay, all criminal charges and levies are waived and you will never have to pay it (as it is written now). I suppose they'll send you letters or phone calls, but that is the extent of what the gov can do to you.

Congress is currently covered under the Federal Employee policy, a group health insurance plan not unlike other large employers'. The policy is around $1100 per month of which the employee currently pays 30% of the $1100. Congress will receive the 70% currently paid to purchase a policy on an exchange. They are not exempt.
